SUPPORTED LIVING SERVICES

Supported Living services assist the participant to learn self-direction and problem solving related to performing and maintaining activities of daily living, instrumental activities of daily living, and engage in community-based activities of the participant’s choosing within his or her personal resources Supported Living Services
● Support the person to live as independently as possible in the community ○ Learn self-direction and problem-solving related everyday life activities ○ Engage in community-based activities of the participant’s choosing within the participant’s personal resources ○ Assistance with finding an apartment or home, and roommates, if needed
● Services are provided in the participant’s own home or apartment
● Supported Living (and the applicable rates) includes nursing case management and delegation services ○ Additional hours may be authorized, based upon the person’s needs, based upon the person’s needs, documented in the person-centered plan and Health Risk Screening Tool. ○ Any delegation of nursing tasks by a licensed registered nurse to a direct support staff (certified by the Maryland Board of Nursing as a Medication Technician) must comply with all applicable laws, regulations (e.g., COMAR 10.27.11), and other requirements.
● Supported Living (and the applicable rates) includes reasonable and local transportation services. The DDA Provider is responsible for providing and arranging for transportation to, from, and within the Community Living Services. The DDA provider shall use the most appropriate and least costly mode of transportation, with priority given to the use of public transportation when appropriate.
● The amount of supervision a person receives is based upon their needs and maybe up to 24 hours, as reflected in the person-centered plan. Dedicated support hours are available, based on the person’s assessed needs and as reflected in the person-centered plan Requirements for Homes
● Supported Living services take place in the person’s own home (not a licensed residence) ○ The person must have a legally enforceable lease or tenancy agreement ○ The person is responsible for their own rent, food, and other living expenses
● The home must meet the requirements of the federal Home and Community-Based Settings Rule.
● The person can live with up to 3 other people that they choose including roommates who may or may not receive DDA services
